La scelta del volume del mdc è legata, da un lato, alla valutazione della corporatura del paziente, dall’altro alla rapidità d’acquisizione delle apparecchiature a 16 e 64 file di detettori. Nello studio angio-TC dell’aorta addominale sono state applicate strategie differenti per ottimizzare questo parametro. Sia l’impiego di un volume fisso, [2] che la somministrazione di 2 ml/kg di peso corporeo [3] si traducono in volumi complessivi elevati, probabilmente eccessivi per lo studio dell’area – limitata lungo l’asse z – della coronaro-TC.
Enhancement vascolare ottimale può essere inoltre ottenuto interrompendo l’iniezione quando all’interno del vaso target viene raggiunta la densità soglia scelta per l’inizio dell’acquisizione (vedi bolus tracking). [5] Nella coronaro-TC, tuttavia, il volume è principalmente condizionato dal tempo d’acquisizione (~20 sec a 16 file, 5-11 sec a 64 file) e dalla necessità di far seguire al mdc un bolo di soluzione fisiologica (vedi bolus chaser). Questo fa sì che volumi di contrasto superiori a 80-100 ml risultino in pratica eccessivi; in effetti, nei protocolli più recenti (Tabella 1), il volume medio è di 100 ml (range 70-150 ml).
